Oklahoma EMT, AEMT & Paramedic License Renewal Requirements

Oklahoma EMS licenses are regulated by the EMS Division, under the Oklahoma State Department of Health. Oklahoma requires NREMT certification for personnel licensed after April 2010 and follows the NCCP model. Providers licensed before that date may renew with state-only CE hours.

Two Renewal Pathways

Pathway 1: With Current NREMT (Preferred)

Submit current NREMT certification, renewal application, CPR card, and fee.

Pathway 2: Without NREMT (State-Only CE)

Additional Requirements

  • Current CPR certification (meets or exceeds AHA standards)
  • Current ACLS (Paramedic only)
  • ALS skills review and verification signed by medical control (Intermediate, AEMT, and Paramedic)
  • Line of duty death benefit fee (per Oklahoma statute)

Oklahoma-Specific Notes

  • NREMT mandatory for post-2010 licensees. Personnel licensed after April 1, 2010, must maintain NREMT. Those licensed before may use state-only CE pathway.
  • Medical director skills verification required for ALS providers. Without it, Intermediate or Paramedic licenses may be issued as inactive (cannot provide patient care until an agency medical director verifies skills).
  • Cannot renew after 90 days past expiration. The EMS Division will not issue any renewed certification or license after 90 days from the original expiration date.
  • Fees vary by level: EMT $20, Intermediate/AEMT $25, Paramedic $30 (plus line of duty death benefit fee).
  • CAPCE-accredited courses accepted.
  • High initial training hour requirements — Oklahoma has some of the highest in the US (154 hrs EMT, 1,050 hrs Paramedic).
  • 3-hour child abuse recognition and reporting training required for initial licensure (since January 2015).

How to Renew

  1. Complete NREMT recertification (if applicable) or state CE requirements
  2. Maintain current CPR (and ACLS for Paramedics)
  3. Obtain medical director ALS skills verification (ALS providers)
  4. Submit renewal application with all documentation
  5. Pay the renewal fee plus death benefit fee
